Important information you should know during the 72 hour Ambulatory EEG test:
The “Event Calendar” is to be completed by the patient listing any events experienced during their Ambulatory EEG test. If no events were experienced they must note that there were “No Events”. This form must be returned along with the equipment to the designated office noted by your tech to avoid any delays in the completion of the reports to be prepared by the Physician.
The following questionnaire is to be completed by the Patient and the Technician before beginning the Ambulatory EEG test.
